Healthcare in Sweden 2026: 1177, fees and the 1 450 kr yearly cap
Who is covered and from when, how to choose and register at a health centre, what 1177 is on the phone and on the web, what a visit costs and where the yearly cap kicks in, and the difference between 1177, närakut, akuten and 112.

A fever on a Sunday night, a personnummer that arrived two weeks ago, and no idea whether to call, walk in, or wait until Monday. Sweden has an answer for each of those, and the wrong choice costs six hours in a waiting room and 400 kr.
Who is covered, and from which day
Healthcare is the region's duty towards the person who lives in the region. In practice that means the person entered in the population register, and registration requires an intended stay of at least a year on a permit that allows it.
Before that entry you pay the full cost of your care yourself. A residence permit in your hand does not change it; 1177 says so plainly. The registration date is the date your fees fall to what everyone else pays, which is the strongest argument there is for going to Skatteverket in your first week.
Two routes go round the rule. If you came for temporary work of less than twelve months, register with Försäkringskassan and you are treated at the same fees as anyone insured here. If you are insured in an EU or EEA country, Switzerland or the United Kingdom, your card gives you necessary care at resident fees.
Asylum seekers and people staying without a permit are entitled to care that cannot wait, and anyone under 18 is treated on the same terms as every other child in the region. The adult fees are capped low: at most 50 kr to see a doctor at a health centre, at most 25 kr for other treatment on referral, at most 50 kr for prescribed medicines. Midwifery, maternity care, the child health centre, inpatient care and the health examination cost nothing. Bring the LMA card; the first visit creates a reserve number instead of a personnummer.
Your vårdcentral, and the word listad
Primary care runs through health centres (vårdcentral). Choosing one is called att lista sig, and it settles where you go for appointments, prescriptions, referrals to specialists, sick notes and vaccinations.
Since 1 July 2026 it settles more than that. At a contracted provider, same-day contact and an assessment within three days apply only if you are listed there. Being unlisted no longer just means a longer queue: the guarantee does not run for you at all.
The other two parts are unchanged. A first visit in specialist care within 90 days of the referral, and treatment within 90 days of the decision to treat. If the region misses those, it has to arrange care with another provider at no extra cost to you.
List the children as well. The child health centre (BVC) for under-sixes is usually in the same building as the vårdcentral.
Logging in to 1177 needs a Swedish personnummer and an e-identification, either BankID or Freja+, and from the age of 13 a young person logs in for themselves. With only a coordination number you can still be treated; you simply cannot use the e-services yet.
What a visit costs, and where the 1 450 kr cap stops it
Each region sets its own patient fees, so the figure a friend quotes is their region's and not yours. In Stockholm every open-care visit costs the same 275 kr, whether you see a nurse or a consultant. In Västra Götaland it is 200 kr with a doctor at the centre you are listed at and 300 kr at any other centre. In Skåne it is 200 kr at the health centre and 200 kr for hospital visits other than the emergency department.

The ceiling is national. Open-care fees stop at 1 450 kr in a twelve-month period, and the amount is the same in every region because the law sets it at 0,025 of the price base amount, 59 200 kr in 2026, rounded down to the nearest fifty kronor. A region may decide on a lower figure but not a higher one. Once you reach it you get a frikort for the rest of the period.
Inpatient care sits outside that and has its own limit: at most 130 kr per day of care in 2026.
Four things are outside the ceiling altogether: vaccinations, certificates and health examinations, copies of your medical record, and a missed appointment.
The missed appointment is the one that catches people. In Stockholm it is 400 kr, and the region charges it to the guardian of a child under 18, to people over 85 and to holders of a frikort, unless there is a valid reason — a reminder text that never arrived is not one. Cancel at least 24 hours ahead. Skåne exempts under-20s and people over 85 from its no-show fee. Västra Götaland charges the ordinary fee with a floor of 100 kr, including where the visit itself would have been free.
Children pay nothing for open care in every region, but the cut-off differs: 18 in Stockholm, 20 in Västra Götaland and Skåne, and 1177 says most regions use the twentieth birthday. Dental care is free nationally through the year you turn 19. People aged 85 and over pay nothing for open care anywhere in the country, and that one is written into the law rather than left to the region.

Medicines: four steps to a frikort
Prescription medicines inside the benefit scheme have their own ladder, and it resets with each twelve-month period.

Children and young people under 18 pay nothing for medicines inside the scheme.
The 3 800 kr ceiling has applied since 1 July 2025. A twelve-month period that began before that date ran on the older 2 950 kr ceiling, and the last of those periods ended on 30 June 2026, so everyone is now on the current figure.
Three in the morning: 1177, närakut, akuten and 112
1177 on the phone is a nurse, around the clock, everywhere in Sweden. From a foreign number the line is +46 771-11 77 00. The recorded menu offers information in English on option 5.
Interpreters are a separate and better right. At a care visit you are entitled to a language interpreter free of charge if you do not understand Swedish. Ask for one when you book rather than when you arrive.
112 is for life-threatening situations only: chest pain, difficulty breathing, heavy bleeding, the signs of a stroke, loss of consciousness. Between that and your health centre sit the local urgent care unit (närakut) and the hospital emergency department, and a call to 1177 is what sorts out which of them you need.

A worked example with dates and amounts
Nour was entered in the population register in Stockholm on 3 February 2026 and listed herself at a health centre in Farsta the same week.
On 9 March she sees a doctor there and pays 275 kr. That visit starts her twelve-month period, which runs to 8 March 2027. A specialist appointment in April is another 275 kr, because Stockholm charges the same for every open-care visit. An emergency department visit in June after a fall is 400 kr, and two more appointments in September are 275 kr each.
Her running total would be 1 500 kr, so the last visit is cut short at the ceiling: she pays 1 450 kr in all, and every visit after it is free until 8 March 2027.
In October she forgets to cancel an appointment. That is 400 kr on top, because a missed appointment sits outside the ceiling and Region Stockholm charges it even to someone holding a frikort.

What we do not know yet
There is no national statute making care free for children. Every region does it, but the cut-off is 18 in Stockholm and 20 in Västra Götaland and Skåne, and 1177 only says that most regions use the twentieth birthday. Look up your own region.
The no-show rules diverge and are not always spelled out. Västra Götaland's wording names free care rather than age groups, so whether a child is charged there is not something this article can state.
1177's national phone menu publishes English and no other language. Individual regions may offer more, and we have not verified that.

How much does a doctor's appointment cost in Sweden in 2026?
It depends on the region. Stockholm charges 275 kr for any open-care visit, Västra Götaland 200 kr with a doctor at the centre you are listed at and 300 kr elsewhere, and Skåne 200 kr. Whatever you pay, open-care fees stop at 1 450 kr in a twelve-month period.
What is högkostnadsskydd and how much is it in 2026?
It is the ceiling on what you pay for open healthcare in a twelve-month period: 1 450 kr in 2026. The law sets it at 0,025 of the price base amount of 59 200 kr, rounded down to the nearest fifty kronor, so it is the same in every region. Can I use healthcare in Sweden before I get my personnummer?

What does it mean to lista sig at a vårdcentral, and do I have to?
It means choosing the health centre you belong to. It is not compulsory, but since 1 July 2026 the same-day contact and three-day assessment parts of the care guarantee apply at a contracted provider only if you are listed there, so being unlisted now costs you the guarantee and not just a place in the queue.
Do I have to pay if I miss a doctor's appointment in Sweden?
In most regions yes, and the fee is outside the högkostnadsskydd. Region Stockholm charges 400 kr and charges it even to guardians of children under 18, people over 85 and holders of a frikort. Skåne exempts under-20s and over-85s. Cancel at least 24 hours ahead.
